Ultrasound images are relatively low resolution, so they look grainy. Also, the field of view (the amount of the body that is visible) is much smaller than for most other imaging studies, so you don't have many adjacent structures that you can see to find your orientation. We depend on labels types on the images to identify what part of the body we are seeing.
