The simplest thing would be to do a cardiac echo, which would show if this is heart or pericardium, and could also serve to guide a tap (for fluid) for both diagnosis and symptom relief. If there is concern for aneurysm, dissection, or rupture, MRI might be considered vs. CT scanning, depending on the level of acuity of symptoms.
